초록
말기 신부전으로 3개월 전에 신장 이식술을 시행 받고 면역억제제를 복용 중이던 54세 여자 환자가 기침과 발열을 주소로 내원하였다. 지역사회 획득성 폐렴을 의심하여 levofloxacin과 amikacin을 투여한 후 환자의 증상과 방사선학적 소견이 호전되어 퇴원하였고 이후 2개월이 지나 다시 증상이 재발하여 재입원하였다. 객담 항산균 도말검사 및 결핵균 PCR이 음성이었으나 기관지 세척액에 대한 결핵균 PCR이 양성으로 나와 결핵을 진단한 후 항결핵제 치료를 시작하였고 이후 객담 및 기관지 세척액에 대한 항산균 배양 검사 상 모두에서 결핵균이 자랐고 치료 후 환자의 증상과 방사선학적 소견을 모두 호전시킬 수 있었다.
