PCXMC 소프트웨어를 이용한 소아에서의 CBCT 환자선량 평가

Abstract

Objective: The purpose of the study was to calculate the effective and absorbed organ doses of cone-beam computed tomography (CBCT) in pediatric patient using personal computer-based Monte Carlo (PCXMC) software and to compare them with those measured using thermoluminescent dosimeters (TLDs) and anthropomorphic phantom. Materials and Methods: Alphard VEGA CBCT scanner was used for this study. A large field of view (FOV) (20.0 cm x 17.9 cm) was selected because it is a commonly used FOV for orthodontic analyses in pediatric patients. Ionization chamber of dose-area product (DAP) meter was located at the tube side of CBCT scanner. With the clinical exposure settings for a 10-year-old patient, DAP value was measured at the scout and main projection of CBCT. Effective and absorbed organ doses of CBCT at scout and main projection were calculated using PCXMC and PCXMCRotation software respectively. Effective dose and absorbed organ doses were compared with those obtained by TLDs and a 10-year-old child anthropomorphic phantom at the same exposure settings. Results: The effective dose of CBCT calculated by PCXMC software was 292.6 μSv, and that measured using TLD and anthropomorphic phantom was 292.5 μSv. The absorbed doses at the organs largely contributing to effective dose showed the small differences between two methods within the range from –18% to 20%. Conclusion: PCXMC software might be used as an alternative to the TLD measurement method for the effective and absorbed organ dose estimation in CBCT of large FOV in pediatric patients.

Abstract

Odontogenic anomalies can occur as a result of conjoining defects. These include fusion, gemination and concrescence. The presence of fused teeth can lead to various clinical problems, including the aplasia of permanent successor, the supernumerary teeth and delayed eruption of permanent teeth. In general, the supernumerary teeth associated with fusion is mainly found in the maxillary anterior region. This report describes rare cases of supernumerary tooth associated with fused teeth of mandibular primary lateral incisor and canine. In the first case, fused teeth was extracted. The supernumerary teeth was erupted at canine space. The second case is still being observed. Extraction of fused teeth and observation of supernumerary teeth is planned.

Abstract

This study reports the unusual complications of 22-year-old male who presented with sudden hearing loss after the right mandibular third molar extraction under local anesthesia with 3.6 ml of 2 % lidocaine. Total 8.75 mg of oral dexamethasone for 1 week immediately after extraction was prescribed in department of oral and maxillofacial surgery but hearing did not improve after 1 week. As referral to otolaryngology, total 600 mg of oral methylon and hyperbaric oxygen therapies were operated for 2 weeks. The hearing of patient was improved at 6 weeks after extraction but tinnitus was persisted even after 12 months. The reason and treatment were discussed with literature review, searching with the keywords [‘hearing loss’ AND (‘dental’ OR ‘tooth extraction’OR‘teeth extraction’)] in PubMed and Google scholar at October 2019. Total five cases were reported after tooth extraction with local anesthesia. The sudden hearing loss could be associated with local anesthesia containing vasoconstrictors. Early steroid (extensive medication and intra-tympanic injection) and hyperbaric oxygen therapies were recommended within 2 weeks. As a proper treatment, hearing could be improved but other additional symptoms, such as tinnitus, dizziness, might be remained.

올세라믹 심미 수복재료의 최신 동향과 적합한 시멘트 선택 및 사용

Abstract

Recently, there are much improvement in optical and mechanical properties of dental ceramic materials cou-pled with improved fabrication techniques, which have caused a considerable shift in the preference of the dentists to ceramic restorations. Because the chemical composition and microstructure of all-ceramic materi-als are different by the type, correct choice of cement type and surface treatment procedure, and cementation strategy is essential for the success of ceramic restorations with adequate retention and decreased incidence of complications. This manuscript reviews on the most often prescribed and some newly developed ceramic materials, and the selection criteria and usage guidelines of cement materials that are used in conjunction with various ceramic materials. This manuscript emphasizes that continuous updating the information of newly developed ceramic and cement materials and application techniques by the dentists and dental staffs are demanding in response to the constantly improving ceramic and cement materials and corresponding application protocol changes.

심미수복을 위한 글라스-세라믹 재료의 치과 응용

Abstract

Porcelain is the first ceramic material to be introduced into dentistry. Porcelain jacket crown was introduced by Dr. Charles H Land in 1886, which was an excellent aesthetic dental restoration but has not been widely used due to high firing shrinkage and low tensile strength. Then metal-ceramic system, which combines the esthetic properties of ceramics and the mechanical properties of metals, was introduced and nowadays it is still used in dental clinical field. However, the metal-ceramic system has shown some problems, such as increased lightness by reflection of light at opaque layer, shadow beneath the gingival line due to the block-out of light by metal coping, exposure of metal in margin part, bond failure between metal and porcelain, oxidation of metal coping during firing the porcelain, etc. Recently, along with the advance of fabrication methods of dental ceramics, the all-ceramic restorations with high esthetic and mechanical properties has increased and gradually replaced metal-ceramic restorations. Especially, CAD/CAM technology has opened a new era in fabricating the dental ceramic restorations. This overview will take a look at the past, present and future possibility of the dental ceramic materials.

Abstract

The esthetic properties of zirconia receive increasing attention as its demand and application escalates. Thus, it is crucial to investigate how zirconia is esthetically different from other dental ceramics. In this study, we evaluated the translucency of zirconia and suggest a method to increase its translucency. We examined the shade of zirconia and offer its distinguishing features. Finally, we analyzed monolithic zirconia restorations to propose esthetic considerations in clinics.

In addition to extensive research on polymer and metal three-dimensional (3D) printing, ceramic 3D printing has recently been highlighted in various fields. The biggest advantage of 3D printing has the ability to easily create any complex shape. This review introduces the 3D printing technology of ceramics according to the type of material and deals with the latest related research in the industrial field including the biomedical engineering field. Finally, the future of ceramic 3D printing technology available in dentistry will be discussed.
