간병 경험이 통증에 대한 주의편향, 통증파국화 및 정확성 추정에 미치는 영향

Effect of caregiving on attentional bias to pain, pain catastrophizing, and accurate pain estimation

초록

적절한 간병을 통해 만성통증 환자를 돕는 것은 최선의 치료를 위해 필수적이다. 그러나 간병인은 통증과 같은 부정적 정서에 노출되어 심리적 고통과 부적응적 행동을 보임에도 불구하고 이 영역에 대한 국내 연구는 충분하지 않은 실정이다. 본 연구의 목적은 인지적 관점에서 간병 경험이 통증을 지각하는 과정에 미치는 영향을 확인하는 것이다. 통증을 동반하는 질병의 환자를 간병한 경험이 있는 (3주 이내 혹은 1년 이상은 제외) 실험집단 24명과 간병한 경험이 없는 통제집단 24명을 대상으로, 통증 및 행복 얼굴표정에 대한 탐침탐사과제를 실시하여 주의편향 지수를 산출하였고, 통증파국화 척도를 작성하였으며, 통증과 중립 얼굴표정을 몰핑한 자극(통증 100/50/40/30%)에 대한 통증 강도의 정확성을 추정하였다. 실험집단은 통제집단에 비해 높은 수준의 통증 주의편향을 보였으며, 또한 실험집단은 행복 얼굴표정보다 통증 얼굴표정에 주의를 더 기울였다. 통증파국화 점수도 실험집단이 통제 집단보다 더 높은 점수를 보였지만, 통증 강도의 정확성 추정에서 두 집단 간에 유의미한 차이를 보이지 않았다. 이상의 결과는 간병 경험이 통증 주의편향의 민감성과 통증파국화 수준을 증가시키는 반면, 통증의 정확성을 추정하는 것에는 영향을 미치지 않음을 의미한다. 후반부는 본 연구 결과의 의의와 한계를 살피고 향후 연구를 위해 제언하였다.

Abstract

This study aimed to identify the effect of caregiving on the process of perceiving pains. An experiment was conducted with 24 caregivers who hadcared for patients with painful diseases and 24 controls. The two groups are compared in terms of their attentional biases towards painful or happy faces, and they completed the pain catastrophzing scale and scored the pain intensisy of morphed faces. The experimental group showed greater bias towards painful faces than the control group. Painful faces also was greater bias compared with happy faces in the experimental group, but not in the control. Moreover, the experimental group reported higher scoreson the pain catastrophizing scale compared with the control group. No significant difference was observed between the two groups in the estimationof pain intensity. These results add to the growing weight of evidence suggesting that pain perception such as bias tendency to pain-related stimuli, is observed in caregivers.
