완전 무치악에서 즉시 부하시와 표준 부하시 임플란트 생존율에 대한 비교 분석: 후향적 연구

Comparative analysis of immediate functional loading and conventional loading about implant survival rate in the completely edentulous: Retrospective study

초록

치근형 티타늄 임플란트를 이용한 치료는 결손된 치아를 수복하기 위해 다양한 증례에 성공적으로 사용되어 왔으며, 따라서 이제는 표준적인 치과 치료 방법의 하나로 자리잡게 되었다1, 2). 1981년 Branemark등1) 은 임플란트 골유착의 성공을 위해서는 치유 기간 중 미세 동요를 줄임으로써 일차 안정을 증진시키는 것이 필수적이라고 하였으며, 이를 위해 임플란트 식립 후 3~6개월간 부하를 가하지 말아야 한다고 하였다. 이러한 부하의 프로토콜을 표준 부하라고 하는데, 이 때 환자들은 식립 후 부하를 가할 때까지의 기간 동안 치아가 결손된 채로 지내거나 임시 가철성 보철물을 장착해야만 하기 때문에 저작 기능, 발음, 심미 등의 관점에서 많은 불편을 감수해야만 한다. 더구나 완전 무치악, 그 중에서도 하악의 완전 무치악 수복 환자에서는 저작이 가능한 자연치가 존재하지 않으며 가철성 임시 보철물은 부족한 유지 및 지지력으로 인해 잘 기능하지 못한다.

Abstract

Introduction: This study researched the clinical application of immediate functional loading technique about implant survival rate in the completely edentulous and suggested our protocol about surgical & prosthetic procedure. Materials and methods: 32 consecutive patients participated in this study. A total of 242 implants were inserted. In test group, 144 implants were inserted in 19 patients with immediate functional loading with occlusal contact. In control group, 98 implants were inserted in 13 patients and conventional loading was done after 4~6 months. Result: In the immediately loaded implants group, overall cumulative survival rate was 98.6%.
