Abstract

Objective : The purpose of this study was to analyze the origin of Meretrix. Method : Bibliographies about Meretrix were examined focusing on the origin. Based on this analysis, the list and characteristics of the origin-mineral were presented. Results : Based on the Meretrix, the Korean and foreign literature were investigated to make a list, and the process of changing the origin was organized in chronological order. Based on these results, the shape of the currently cultivated or distributed drugs, the status of the distributed drugs and the status of the distributed products were summarized. In addition, the results of summarizing the adverse effects and sensitivities of the efficacy and prestige are as follows: 1. The origin of the Meretrix corresponded to the fall of the mollusks Merethrix metrix L. and Cyclina sinensis Gmelin, which are mollusks belonging to the reference family Veneridae. Their Herbal name is CONCHA CYCLINAE SEU MERETRICIS and they have therapeutic effects on cough, asthma, thirsty and chest heavies. 2. Munhap (文蛤) is a different name of GALLA CHINENSIS. It has therapeutic effects on various skin diseases.Conclusions : The Meretrix means two herbs. The first one is CONCHA CYCLINAE SEU MERETRICIS, the other is GALLA CHINENSIS. They should be used separately according to their clinical use.

Abstract

Objective: The purpose of this study was to report the improvement of two children with attention deficit hyperactivity disorder (ADHD) following herbal treatment according to the provision of Shanghanlun. Methods: We diagnosed patients on methylphenidate for ADHD as no. 26 provision in Taeyangbyung, and analyzed the improvement from the medical records, teachers' statements, The Korean Version of Inattentive/overactive with Aggression-Conners Rating Scale(K-IOWA CRS). Results: We found a change in the symptom after Baekhogainsam-tang administration. In the first case, the K-IOWA-CRS score changed from 12 to 8 during the previous 70 days of treatment, however, surged to 21 without Baekhogainsam-tang treatment. The score stabilized to 8 after 49 days later. In the second case, the score changed from 9 to 3 following Baekhogainsam-tang treatment. Conclusions: These cases show behavioral improvements with Baekhogainsam-tang treatment in ADHD children, who were diagnosed with no. 26 provision according to Shanghanlun.

Abstract

Objective : The purpose of the present study was to report the improvement of functional abdominal pain in a patient treated with a herb medication based on a disease pattern identification diagnostic system under the provision of Shanghanlun (DPIDS). Methods : According to DPIDS, the patient was diagnosed with Taeyang-byung, no. 25 provision, and was administered with Gyejiyimahwangyil-tang herb medication for 60 days. The severity of the abdominal pain was assessed with the five-point Likert scale. Results : The abdominal pain in the patient disappeared following treatment with Gyejiyimahwangyil-tang, which is described in the 25th provision of Shanghanlun. Conclusions : The present case report suggests that ‘瘧’, a word in the 25th provision of Shanghanlun may indicate severe abdominal pain at nighttime and not just malaria.

Abstract

Objective : The purpose of this study was to report the improvements of patients with globus pharyngeus treated with herbal medication from Shanghanlun. Methods : Each patient was diagnosed with Greater-yang disease 29th provision of Shanghanlun, and Gamchogungang-tang was used to treat. The result was evaluated by Glasgow-Edinburgh Throat Scale(GETS). Results : The GETS score decreased from 23 to 4 after 40 days of administration in case 1 and from 31 to 1 after 15 days of administration in case 2. Conclusions : Globus pharyngeus improved in each case. This case report suggests that ‘急’ based on the 29th provision of Shanghanlun, can be a direct psychological cause of globus pharyngeus.

Abstract

Objective : This study was performed to report two cases diagnosed with the Shanghanlun disease pattern identification diagnostic system (DPIDS) and treated with Jeodang-tang. Methods : Based on DPIDS, we analyzed two cases treated with Jeodang-tang. Results : In the first case, the bipolar depression rating scale (BDRS) score improved from 44 to 13. In the second case, the fatigue severity scale (FSS) score improved from 55 to 24. Conclusions : In Shanghanlun, Jeodang-tang may not only treat pattern/syndrome of blood stasis but also many other diseases.

Abstract

Objective : This case report aimed to demonstrate the effect of Baekhogainsam-tang on hypersomnolence. Methods : A 33-year-old woman complained of hypersomnolence with fatigue and lack of concentration in the daytime. According to the diagnostic system based on Shanghanlun provisions (DPIDS), the patient was treated with Baekhogainsam-tang decoction. The result was evaluated by the Epworth Sleep Scale. Results : After administration of Baekhogainsam-tang decoction for 30 days, the Epworth Sleep Scale score was decreased from 15 (pathologic sleepiness) to 1 (no clinically significant sleepiness). Conclusions : The patient completely recovered from hypersomnolence and fatigue following treatment with Baekhogainsam-tang decoction according to DPIDS.

Abstract

Objective : This case report presents the effect of Hoiyeok-tang on atopic dermatitis. A patient was diagnosed with Lesser Yin disease based on Shanghanlun provisions. Methods : According to the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS), we diagnosed the patient with Lesser Yin disease. The patient was treated with Hoiyeok-tang for 87 days. The changes in symptoms were estimated by the SCORAD index. Results : After treatment, the SCORAD Index improved from 57.3 to 33.5. The patient also body weight from 89.1 kg to 76.5 kg. Conclusions : This case study shows the effectiveness of Hoiyeok-tang on atopic Dermatitis according to DPIDS.

Abstract

Objective : There was an improvement in a patient with psoriasis following the intake of Hoeyeok-tang for 317 days. By analyzing these cases, we reviewed the use of Hoeyeok-tang in skin diseases and attempted to interpret the related Shanghanlun articles. Methods : The diagnostic process was analyzed according to the disease pattern identification diagnostic system based on the Shanghanlun provisions. The progression of psoriasis, which was addressed, was assessed using the PASI score. Results : The patient was diagnosed with Soeum-byung 324, and the PASI score decreased from 13.6 to 0.4 after the intake of Hoeyeok-tang for 317 days. The patient's compliance and expectation from treatment were good, and no specific adverse effect was identified. Conclusions : This study suggests the clinical use of Hoeyeok-tang in psoriasis based on Shanghanlun.

Abstract

Objective : The purpose of this study was to report the improvement in a patient with cystitis by a herbal medication based on Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods : A patient was diagnosed with cystitis according to DPIDS. The patient was treated with Galgunhwangryunhwanggum-tang. Results : The symptoms of cystitis disappeared after treatment and did not recur after 4 months. Conclusions : The herbal medication Galgunhwangryunhwanggum-tang is effective in this case of cystitis.

Abstract

Objective : The purpose of this study was to report the improvement in a patient with edema treated by herb medication based on Shanghanlun provisions. Methods : According to ‘A disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS)’, a patient was diagnosed with Taeyang-byung, no. 46 provision and was administered with Mahwang-tang herbal medication for 30 days. The Numeric rating scale (NRS) was used to estimate the response. Results : The NRS score changed from 10 to 1 and the edema disappeared by 46th provision of Mahwang-tang selected according to Shanghanlun provisions. Conclusions : This case report suggests that the two words in the 46th provision of Shanghanlun, ‘陽氣重, 無汗’ (Yang qi heavy, Absence of sweating) indicate a close relationship between photosensitivity and absence of sweating, thereby affecting edema in this case.

Abstract

Objective : The purpose of this study was to report the improvement in two patients with different complaints of sleep disorder treated with herbal medication based on the Shanghanlun disease pattern identification diagnostic system (DPIDS), and hypothesize the meaning of word ‘更’. Methods : According to DPIDS, patients were diagnosed with Taeyang-byung no. 29 provision, and took Gamchogungang-tang herb medication 15 or 90days. The results of administration were evaluated by the hyperhidrosis disease severity scale (HDSS) and the visual analog scale (VAS). Results : After administration of Gamchogungang-tang 15 or 90days, based on 29 provision of Gamchogungang-tang diagnosed according to Shanghanlun provisions, in one case the HDSS decreased from 3 to 1 and the VAS decreased from 10 to 1. In the other case, the VAS decreased from 10 to 1. Conclusions : Each patient not only recovered from symptoms of hyperhidrosis and alopecia areata, but also from sleep disorders following administration of a single medication of Gamchogungang-tang. This case report suggests that the word ‘更’ in the 29 provisions of Shanghanlun means sleep disorder in this case.

Abstract

Objective : This study was intended to evaluate if treatment with Osuyu-Tang following the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS) worked analyzing the progress of patients with migraine and restless legs syndrome and to reinterpret the provision of 309 Soyinbing Osuyu-Tang based on the analysis of whether it worked. Methods : Two cases treated with Soyinbing Osuyu-Tang were analyzed using DPIDS. The migraine and restless legs syndrome in both cases were evaluated using the migraine disability assessment (MIDAS) and the Korean versions of the international restless legs scale (K-IRLS), respectively. Results : In the first case, during 45 days of treatment, the MIDAS decreased from 10 days to 3 days. In the second case, during 30 days of treatment, the K-IRLS decreased from 30 to 9. In both cases, Soyinbing was associated with the disease, although the activity was less; and Leng (冷) was associated with numbness in the kneeling position. Conclusions : It was confirmed that treatment with Osuyu-Tang was effective against migraine and restless legs syndrome and that Leng could be associated with numbness in the kneeling position. The etymological analysis of the Shanghanlun characters suggested the possibility of further clarifying the clinical significance and interpretation of Shanghanlun.

Abstract

Objective : The purpose of this study is to report the improvement of patient with Dyshidrotic Eczema treated by herbal medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to ‘Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions’, the patient diagnosed as ‘Jueyinbing Huoluan’ number 388 provision and took herbal medication Hoiyeok-tang for 28days. The changing symptom progress of Dyshidrotic Eczema was evaluated by DLQI(Dermatology Life Quality Index) and VAS(Visual Analogue Scale). Results : The DLQI score changed 24 to 0 and VAS changed 8 to 0. Conclusions : Patient who suffered from Dyshidrotic Eczema caused by concentrating on sophisticated work using her eyes and hands was treated by Hoiyeok-tang in Shanghanlun. Skin disease on hands or feet can be closely related to conception of 手足厥 in Shanghanlun. Further studies are needed to validate the conception of 手足厥 and the effects of Hoiyeok-tang.

Abstract

Objective : To explore the meaning of Eumyangyeokchahunobok-byung (陰陽易差後勞復病) through two cases and finding the relationship with the DSM-5 sleep-wake disorder. Methods : Following a disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS), we analyzed cases with chronic fatigue and diplopia caused after sleep disorders. The patients were treated with the Soshiho-tang and Yijoong-tang. Results : The two herbal medicines showed remarkable improvement in the patients. The sleep disorders and the secondary pathologies were rapidly relieved within a month. Conclusions : The patterns of sleep disorders of the two patients were found to be very similar to the circadian rhythm sleep-wake disorders, as described in DSM-5. Although the etymology of the Chinese characters has not been clearly known yet, it was possible to present the original hypotheses on 更and 了due to clinical inference.
