치과용 수복물은 치아의 손상된 기능 회복 뿐 아니라 적절한 심미성을 재현할 수 있어야 한다. 특히 심미성에 대한 관심이 증가하면서 전치부 뿐 아니라 강한 응력을 받는 구치부에도 치과용 합금 보다는 심미성이 우수한 세라믹을 적용하려는 추세로 고강도 세라믹인 지르코니아의 응용 범위가 크게 증가하고 있다. 최근 Cercon (DeguDent, Germany), LAVA (3M ESPE, USA) 및 Procera AllZirkon (Nobel Biocare, Geermany) 등과 같은 치과용 지르코니아 시스템들이 소개되고 있지만, 이러한 지르코니아 시스템을 치과용 수복물로 적용할 때 안정적인 내구성 유지를 위하여 적절한 결합력은 필수적이다. 따라서 새로운 치과용 세라믹의 개발과 함께 신소재 세라믹의 결합력을 향상시킬 수 있는 다양한 방법들이 소개되고 있다. 임상에 적용한 치과용 세라믹 수복물의 성공은 수복물과 치질 (tooth structure)의 결합을 형성시키는 접착 술식에 영향을 받는다. 현재까지 임상에서 사용하고 있는 합착용 시멘트 (luting cements)는 인산아연 시멘트, 폴리카복실레이트 시멘트, 글라스아이오노머 시멘트, 레진강화형 글라스아이오노머 시멘트 및 레진 시멘트 등 5 가지로 분류하고 있다. 합착용 시멘트에 관하여 광범위하게 연구가 진행되었지만, 적절한 시멘트의 선택 기준은 아직 명확하지 않은 실정이으로 대부분의 경우 우선 각 시멘트의 장‧단점을 고려하고, 접착할 수복물의 디자인 (단순 유지 또는 접착), 수분 조절 및 수복물의 재질 등을 분석하여 그 상황에 가장 적절한 시멘트 선택을 추천하고 있다. 치질과의 강한 결합은 치과용 세라믹 수복물의 유지력과 변연 적합도를 향상시킬 수 있어서 임상 성공에 매우 중요하다. 지르코니아 수복물도 안정적인 접착력 유지가 수복물 내구성에 큰 영향을 줄 수 있는데, 다른 치과용 세라믹과 비교하여 지르코니아의 낮은 결합력은 해결하여야 할 중요한 문제점으로 지적되고 있다. 초기에는 인산아연 시멘트나 글라스아이오노머 시멘트로 지르코니아 수복물을 합착시켜 사용하였지만, 최근에는 변연 봉쇄능과 유지력이 우수하고 파절 저항성을 증가시킬 수 있는 레진 시멘트 적용이 증가하고 있다. 지르코니아에 대한 결합력은 지르코니아의 표면 거칠기와 표면 에너지 및 접착제의 적심성 (wettability) 등과 같은 다양한 요소에 영향을 받을 수 있다. 상용중인 지르코니아 제품들은 제조공정에 따라 지르코니아 표면에 독특한 음각 (intaglio)이 형성될 수 있기 때문에 각 제품에 따라 또는 표면 처리법에 따라 결합력에 큰 차이가 있을 수 있으며, 다양한 레진 시멘트 중에서는 지르코니아에 대한 적심성이 우수한 시멘트가 지르코니아와의 결합력도 우수하다고 한다. 따라서 본 논문에서는 치과용 지르코니아의 표면처리 방법, 프라이머 종류 및 레진 시멘트 성분 등이 지르코니아의 결합력에 주는 영향에 관하여 알아보고자 하였다.
