사람들이 갖는 치과 치료에 대한 불안과 공포는 보편적인 감정으로서, 특히 대다수의 환자들이 치과 내 소음, 마취 주사, 발치 등의 물리적인 자극뿐만 아니라 치과치료를 받는다는 심리적인 자극만으로도 불안과 공포를 느낀다고 알려져 있다. 또한, 이러한 불안과 공포의 존재만으로도 치과 치료 후 느끼는 통증의 양이 증가된다고 보고 되고 있어, 환자들의 삶의 질(quality of life) 향상을 위해서는 불안과 공포를 적극적으로 예방하고, 통증을 야기할 수 있는 물리적 및 심리적인 원인을 근본적으로 차단할 필요가 있다. 수술과 연관된 통증 조절을 위해서 현재 많은 진통제나 신경안정제가 개발되고 임상에서 사용 되고 있다. 특히, 복합 처방된 진통제의 효용성은 여러 논문을 통해 잘 알려져 있으며, 그 중에서도 Mypol(성원에드콕, 대한민국)은 Codein phosphate 10mg, Acetaminophen 250mg, Ibuprofen 200mg이 복합 처방된 진통제로서 조 등에 의하면 단순히 nonsteroidal anti-inflammatory drugs와 taminophen을 두 배로 처방할 때 보다 더 큰 진통효과를 얻을 수 있으며, 부작용이 적고 작용시간이 길어 임상에서 많이 사용되고 있다고 하였다. 본원 에서도 현재 임플란트 수술 전 예방적 항생제와 함께 Mypol을 투여하고 있으며, 이외에도 치은이식수술, 치조골이식술 등 침습도가 높은 수술 후 통증 조절을 위해 효과적으로 사용해오고 있다. 하지만 전술한 복합 진통제(Mypol)의 처방에도 불구하고, 수술에 대한 환자들의 공포는 여전히 조절되지 않고 있으며, 특히 약물 처방에 의한 실질적인 통증경감효과를 파악하기 어려운 경우가 많아 환자 통증관리에 어려움이 존재하는 실정이다. 따라서 본 연구에서는 통증을 최소화하는 환자 중심의 임플란트 술식을 개발하고자 임플란트 식립 환자에서 기질 불안을 측정하여 개개인의 통증 민감도를 분석한 뒤, 수술 후통증의 변화를 48시간 동안 추적 관찰하여 물리적 자극 외에 심인성 자극이 통증에 끼치는 영향에 대한 분석을 시행하였으며, 또한 임플란트 수술과 연관된 물리적 통증 외의 기질이나 심리 또는 환자의 성별과 연령, 이전 치과 치료 경험, 수술 시간, 부가적 수술 유무 등 통증에 영향을 줄 것으로 의심되는 요소들의 실제 영향을 평가하여 보고자 하였다.
