본 연구에서는 성인의 아동기 성학대 외상경험과 정신화의 관계에서 외상경험 개방에 대한 정서적 지지의 조절효과를 검증하였다. 본 연구를 위해 2017년 7월부터 2018년 1월까지 전국의 19세 이상 성인 남녀 965명을 대상으로 조사하여 아동기 성학대 외상경험이 있는 것으로 보고된 235(24.3%)명의 자료를 분석에 사용하였다. 분석에 사용된 질문지는 성학대 외상경험 질문지, 정신화 척도, 외상경험 개방에 대한 정서적 지지반응 척도로 구성하였고, 성인의 10명 중 2.4명은 아동기 성학대 경험이 있는 것으로 나타났다. 위계적 회귀분석 결과, 아동기 성학대와 정신화 및 정신화 하위영역의 관계에서 아동기 성학대의 주 효과는 유의하였고, 외상 경험 개방에 대한 정서적 지지반응 수준에 따른 정신화, 정서인식 실패 및 경직된 사고가 달라지는 조절효과가 있는 것으로 나타났다. 한편, 정서적 지지반응 수준에 따른 차이에서 아동기 성학대 고집단에 미치는 영향은 미미하거나 경직된 사고는 더 높아져, 단순한 정서적 지지반응만으로 정신화가 변화되기란 어렵다. 본 연구 결과를 바탕으로 아동기 성학대가 정신화 발달에 부정적 영향을 미치는 요인으로 볼 수 있으며, 성학대 외상경험은 전문적인 치료적 관계 안에서 안전기지를 확보하여 개입해야 한다는 것을 시사한다.
The purpose of this study was to examine the moderating effect of emotional support reaction to the disclosure of a traumatic experience on the relationship between childhood sexual abuse (CSA) experiences of trauma and its mentalization. The sample included 965 adults, with data from the subset of 235 adults who had experienced CSA being analyzed. Approximately 2.4 out of 10 adults in the sample had experienced CSA. The main effect of CSA was significant on the mentalization both at overall and subdomain levels. The level of emotional support had a buffering effect that modulates mentalization, deficit of affection awareness and concrete thinking. The results of the study indicate that CSA is an influential factor that negatively affects the development of mentalization. CSA should be treated in a professional therapeutic relationship to properly address mentalization. Current approaches for adult counseling for CSA, implications and limitations of the study are discussed.
