본 연구의 목적은 학교폭력이 복합 외상의 특성을 보이는지 확인하고, 복합 PTSD 진단을 적용하여 단순 PTSD가 포함하지 못하는 학교폭력 피해 청소년들의 증상을 알아보는 것이다. 이를 위해 일반 중학교, 대안학교, 상담실에서 총 640명을 대상으로 불성실하게 응답한 경우와 피해 경험이 전혀 없는 경우를 제외하고 총 135명의 자료를 분석대상으로 하였다. 학교폭력 피해 보고는 일회적으로 단기간 당한 경우부터 반복적으로 장기간 당한 경우까지 다양하게 나타났고, 후자의 경우에서 복합 외상의 특성을 확인할 수 있었다. 단순 PTSD, 복합 PTSD, 동시진단은 학교폭력을 처음 경험한 연령이나 피해 기간, 발생 빈도, 피해 유형의 중첩과 같은 외상특성에 따른 차이가 나타나지 않았다. 상담장면에서 단순 PTSD로 진단받은 청소년들 중 27.2%는 단순한 불안 증세가 아니라 복합 외상으로 인한 자기조절기능과 성격 문제를 나타내었다. 단순 PTSD로 진단을 받은 집단은 치료적 개입을 받을 수 있게 되지만 복합 PTSD 증상만 갖고 있는 11.1%는 현재까지 치료의 사각지대에 놓여있다. 이 집단은 자기 인식의 변화, 의미체계의 변화에서 높은 증상을 갖고 있기 때문에 심층적인 치료가 필요한 집단임에도 불구하고 그동안 간과되어왔다. 본 연구는 단순 PTSD가 포함하지 못했던 복합 외상으로 인한 자기조절기능과 성격의 변화까지 치료 목표로 삼아 적합하고 다양한 치료가 적용될 수 있는 방향을 제시하는데 의의가 있다.
School violence may put adolescents at risk of suffering not only from post-traumatic stress disorder (PTSD) but may also cause an impairment in affective, cognitive, biological, and relational self-regulation (complex PTSD). The aims of this study were to examine the differences in symptoms among adolescents with simple post-traumatic stress disorder (simple PTSD), adolescents with complex post-traumatic stress disorder (complex PTSD), and adolescents with both simple PTSD and complex PTSD. A total of 640 adolescents took part in the study, and data from 151 adolescents who had reported that they had experienced at least one incident of school violence was analyzed. The subjects were asked to complete a number of self-report measures which examined their experiences of bullying, post-traumatic stress disorder and complex post-traumatic stress disorder. The results of this study were as follows: First, there was no discernable trend in regards to age at the onset of the trauma or the duration of the trauma, the number or type, or level of frequency within the diagnosis. Second, there were significant differences in the features of simple and complex PTSD within the diagnosis. Finally, the implications for future research studies and counseling for victims are suggested.
